What is the Housing Stability Program?

The Housing Stability Program provides short-term support for people who have housing but are at risk of losing it. Participants need to be willing to work with a Housing Stability Worker (HSW) to set goals for staying in their homes.

This program works with participants, community partners, families, and landlords to solve problems, set goals, and create service plans that help individuals stay housed.

Our Goals

  • Support individuals and families so they remain housed
  • Reduce barriers for low-income households
  • Create connections to community services and supports
  • Review all sources of income available and assist with income applications (OAS, CPP, GIS, Pensions)

How can the Housing Stability Program help me?

Our Housing Stability Workers support vulnerable, low-income individuals and families throughout Chatham-Kent who have a housing stability concern, as well as provide support to anyone living in a Private Non-Profit (PNP) building.

The individuals and families referred to this program must be:

  • 18 years of age or older
  • Low-income
  • Residents of Chatham-Kent
  • ODSP participants
  • Currently housed but may be facing housing instability


Housing Stability Program Referral Process

Complete a Housing Stability Program referral form and email it to ckintake@chatham-kent.ca or print and fax it to 519-351-5090.

The referral will be assigned and reviewed for eligibility to the program within 5 to 7 days. Depending on program availability, the referral may be added to a waitlist and contacted later.
